It was Isaac's fault. He was the one who shattered the goddamn Black Soulstone.
Sometimes when you're driving down a road, a bug hits your windshield. It's ugly and unpleasant, but you just activate the wipers / cleaning fluid and it's gone in a few seconds. It's just part of driving down the road. You don't dwell on it, because that'd be silly. Bugs aren't worth the time.
You just discard their guts and keep on driving.

I thought it was because Malthael broke it. -_-;
Isaac was going to basically nuke it, due to it only being able to be destroyed by really large explosions. (According to Sariel.)
Either way, Mephisto will be confronted soon enough.

Even if you deal with him, there's still the many other lords of hell. Though we can get to that later.
